Maybe the 4th infirmary is the place where 'incarnation' doing thing with lifespan that they collect from citizen under Big Mom's territory.. AS we knew.. That incarnation is not homies (Big Mom put soul into object and animal and turn them into homies) but it work under Big Mom's control.. Early, we knew that Big Mom pull out lifespan from one of her son named Muscato.. And then Incarnation collects his lifespan.. Other Big Mom's children ask Incarnation to collect the lifespan and send Muscato somewhere.. Maybe the 4th infirmary is the place where they work with lifespan..
If the case that incarnation work in that infirmary.. This is important to Big Mom too.. Because Incarnation work direct under her.. And she doesn't want anyone to disturbing Incarnation to doing their work..
I think beside Muscat.. Cracker also been send there too.. Maybe Pekoms also can be seen there too.. Snack might be there too..
